Statin questions rarely ask you to recite a cholesterol target. They ask whether you can tell benign muscle ache from rhabdomyolysis, whether you spot the interacting antibiotic added to a simvastatin regimen, and whether you know that treatment decisions rest on absolute cardiovascular risk rather than the LDL number alone.

Statins are among the most-dispensed medicines in Australia, which means the interaction that raises statin exposure and the patient reporting muscle aches both walk into a pharmacy constantly. Both are pharmacist-managed problems: one is an interaction check at the point of dispensing a short antibiotic course, the other is a triage decision about whether an aching patient needs reassurance or urgent bloods. Core concepts

Absolute risk, not the number in isolation

For primary prevention — patients without established cardiovascular disease — the decision to start a statin rests on absolute cardiovascular risk calculated from a combination of factors: age, sex, smoking status, blood pressure, diabetes, lipid levels and others. A moderately raised cholesterol in a young non-smoker with normal blood pressure carries a very different implication from the same figure in an older smoker with hypertension.

For secondary prevention — patients with established cardiovascular disease — the calculation is not needed. These patients are already at high risk by definition, and statin therapy is indicated regardless of the baseline lipid level. Recognising which of these two situations a stem describes is often the whole question.

Work from the current Australian cardiovascular risk assessment guidance rather than from a remembered LDL threshold. The specific cut-offs and the risk calculator itself are revised periodically, so the durable exam skill is knowing that absolute risk drives the primary-prevention decision — not memorising a number that may have moved.

Statin intensity

Statins are grouped by the magnitude of LDL reduction they typically achieve rather than by milligram dose, because equivalent doses differ substantially between agents. Higher-intensity therapy generally means atorvastatin or rosuvastatin at higher doses; moderate-intensity covers lower doses of those agents and typical doses of others such as simvastatin and pravastatin.

The practical implication is that you cannot compare statins by dose number — 40 mg of one agent is not equivalent to 40 mg of another. When a stem describes switching between statins, the intensity category is what needs to be matched, not the number.

Muscle effects: a spectrum, not a single event

Muscle symptoms are the adverse effect patients hear about and the one most likely to prompt a pharmacy conversation. They span a genuine spectrum, and the response differs at each point.

Mild muscle aches without significant creatine kinase elevation are common and often manageable — by dose reduction, by switching agent, or sometimes by continuing with reassurance once other causes are excluded. Myopathy with a marked CK rise is more serious and generally prompts stopping the statin. Rhabdomyolysis, characterised by severe muscle pain and weakness with a very large CK rise and dark urine, is rare but a medical emergency because of the risk of acute kidney injury.

The dark urine detail is the one that should change your answer: it signals myoglobinuria and moves the scenario from "review the statin" to "urgent assessment".

  • Risk of muscle effects rises with higher statin doses, with hypothyroidism, with renal impairment, in older patients, and with interacting medicines.
  • Untreated hypothyroidism is a recognised contributor and worth considering in a patient with new muscle symptoms.
  • Strenuous unaccustomed exercise can raise CK independently — the history matters before attributing everything to the statin.

What creatine kinase actually tells you

Creatine kinase (CK) is an enzyme found in muscle, released into the blood when muscle cells are damaged. It is the laboratory marker used to gauge how much muscle injury is actually occurring, which is what separates a nuisance ache from a dangerous one.

  • A normal or only mildly raised CK alongside aches suggests muscle symptoms without significant muscle injury — uncomfortable, but not dangerous in itself.
  • A markedly raised CK indicates genuine muscle damage and points towards myopathy.
  • A very large CK rise, particularly with dark urine, points to rhabdomyolysis, where released muscle contents threaten the kidneys.
  • CK is not statin-specific — vigorous or unaccustomed exercise, trauma and intramuscular injections all raise it, so the result is interpreted alongside the history rather than on its own.

Triaging the patient in front of you

A patient collecting their statin who mentions their legs ache is the most common version of this scenario, and the reflex answers — "stop it" or "that's normal" — are both wrong. What resolves it is a short sequence of questions.

  1. How severe is it, and is there weakness as well as ache? Weakness raises the concern considerably.
  2. What colour is the urine? Dark urine escalates this to urgent assessment immediately.
  3. When did it start, and does it relate to a dose increase or a newly started medicine? A new interacting drug is a frequent trigger.
  4. Has there been unaccustomed exercise, or is there untreated hypothyroidism? Both can explain muscle symptoms or raise CK independently.
  5. Based on that, decide: reassure and monitor, arrange CK testing and review, or refer urgently. Not every ache needs the statin stopped, and not every ache can wait.

Clinical application

The interactions that actually matter

Simvastatin and atorvastatin are metabolised substantially by CYP3A4, so inhibitors of that enzyme raise their concentrations and with it the risk of muscle toxicity. The clinically important inhibitors to recognise are the azole antifungals such as itraconazole and ketoconazole, the macrolide antibiotics clarithromycin and erythromycin, certain HIV and hepatitis C antivirals, and the non-dihydropyridine calcium channel blockers verapamil and diltiazem. Grapefruit juice in substantial quantities does the same thing.

The classic community scenario is a short course of clarithromycin dispensed to a patient stably taking simvastatin. The statin has been safe for years, the antibiotic course is only a week, and nothing looks wrong — which is precisely why it is such a good exam question. Management commonly involves temporarily withholding the statin for the duration of the antibiotic course, or selecting an alternative antibiotic, rather than simply dispensing both.

Pravastatin and rosuvastatin are not appreciably metabolised by CYP3A4, which makes them useful alternatives when an interacting medicine cannot be avoided. Rosuvastatin has its own interactions to check, so "not CYP3A4" does not mean "no interactions".

Fibrates, and why the choice of fibrate matters

Combining a statin with a fibrate increases the risk of muscle toxicity, but not equally across the class. Gemfibrozil interferes with statin metabolism in a way that markedly increases statin exposure and rhabdomyolysis risk, and the combination is generally avoided. Fenofibrate does not carry the same degree of risk and is the preferred fibrate where combination therapy with a statin is genuinely required.

A stem offering both fibrates as options is testing this distinction specifically — the answer is not "avoid all fibrates with statins".

Timing depends on half-life

Simvastatin is conventionally taken in the evening, because its relatively short half-life means aligning the dose with the overnight peak of endogenous cholesterol synthesis improves its effect. Atorvastatin and rosuvastatin have much longer half-lives, so their effect does not depend on time of day and they can be taken whenever the patient will reliably remember.

This makes for a neat exam question because the intuitive answer — "all statins at night" — is wrong, and because the correct answer for the long-acting agents is the one that supports adherence.

The counselling that keeps patients on treatment

Statins are preventive, which makes them uniquely easy to stop: the patient feels no different taking them and no different for a while after stopping. Several of the recurring adherence problems are addressable at the counter.

  • An improved cholesterol result means the statin is working, not that it is no longer needed — stopping it lets the level drift back up. This is one of the most common reasons patients quietly discontinue.
  • Statins complement diet, physical activity and smoking cessation rather than replacing them; the lifestyle measures continue alongside.
  • Muscle symptoms should be reported rather than met with silently stopping the medicine, since many are manageable by dose reduction or a change of agent.
  • Don't stop without advice — particularly in secondary prevention, where the statin is doing ongoing work regardless of how the numbers look.

Add-on therapy and contraindications

  • Ezetimibe reduces cholesterol absorption by blocking the intestinal cholesterol transporter NPC1L1, and is the usual first add-on when a statin alone doesn't achieve the target, or when statin dose is limited by adverse effects.
  • PCSK9 inhibitors are injectable agents reserved for patients such as those with familial hypercholesterolaemia or very high risk who remain inadequately controlled, with access restrictions in Australia.
  • Statins are contraindicated in pregnancy and breastfeeding, and women of childbearing potential should be counselled accordingly — a routine annual dispensing may be the only point where this is raised.
  • Statins are contraindicated in active liver disease and in unexplained persistent transaminase elevation; routine liver function monitoring beyond a baseline is generally not required in the absence of symptoms.
  • Familial hypercholesterolaemia should be suspected with very high LDL levels, a strong family history, or premature cardiovascular disease, and warrants specialist referral and family screening rather than routine management alone.

  • Calculating absolute cardiovascular risk for a patient who already has established cardiovascular disease — secondary prevention doesn't require it.
  • Dispensing clarithromycin to a patient on simvastatin without recognising the CYP3A4 interaction and the resulting muscle toxicity risk.
  • Treating all muscle symptoms identically, rather than triaging mild aches differently from severe pain with dark urine.
  • Missing dark urine as the feature that escalates a scenario from routine review to urgent assessment.
  • Comparing statins by milligram dose rather than by intensity category when a switch is described.
  • Advising every statin be taken at night, including the long half-life agents where timing makes no meaningful difference.
  • Avoiding all fibrates with statins rather than distinguishing gemfibrozil, which is avoided, from fenofibrate, which is preferred.
  • Continuing a statin in pregnancy, or failing to raise contraception and pregnancy planning with a woman of childbearing potential.
