How levothyroxine must be taken
Levothyroxine absorption is reduced by food and by several common co-administered products, so it is taken on an empty stomach — conventionally 30 to 60 minutes before breakfast, with water. The critical practical detail is the lag. The pituitary takes weeks to re-equilibrate after a dose change, so TSH is generally rechecked around six to eight weeks after any change in dose or brand. Testing sooner produces a result that reflects the old dose more than the new one, and acting on that premature result is a recognised route to over-correction. A stem describing a TSH taken two weeks after a dose increase is usually testing whether you know not to act on it.
The OPRA pattern: a stable patient whose TSH suddenly moves
This is the shape most levothyroxine stems take. A patient has been well controlled for months or years, and their TSH is now abnormal. The instinct is to change the dose. The exam is almost always testing whether you look for a cause first.

Reading thyroid function tests
OPRA does not expect endocrinologist-level interpretation, but it does expect you to read a TSH and free T4 pair and know which direction the problem runs. The pattern to hold onto is that in primary thyroid disease, TSH and free T4 move in opposite directions.
Starting levothyroxine: not everyone starts at the same dose
A young, otherwise healthy adult with hypothyroidism can generally be started at or near a full replacement dose based on body weight. Older patients, and particularly patients with ischaemic heart disease, are started at a low dose and titrated slowly upwards.
The reason is cardiac. Restoring thyroid hormone raises metabolic demand and heart rate, and doing that abruptly in a patient with coronary disease can precipitate angina or an arrhythmia. Levothyroxine has a long half-life — on the order of a week — which is why a single missed dose does not produce an immediate clinical effect and why patients should not be alarmed by one. The counselling point is that the medicine is replacing a hormone the body normally holds at a steady level, so what matters is the average intake over time rather than any individual day.
A missed dose is generally taken as soon as it is remembered that day. If it is not remembered until the following day, standard advice is usually to take the missed dose in addition to that day's dose rather than skipping it — the long half-life makes this well tolerated, unlike medicines where doubling up would be dangerous. Confirm the specific wording against a current reference, and treat a pattern of repeatedly missed doses as an adherence issue that will show up as an unexplained TSH result.
Over-replacement has real consequences
It is tempting to treat a slightly suppressed TSH as harmless, or even preferable to under-treatment. It isn't. Sustained over-replacement produces a subclinical hyperthyroid state associated with atrial fibrillation and with reduced bone mineral density and increased fracture risk, both of which matter most in exactly the older population where over-replacement is most likely to be tolerated symptomatically.
Symptoms of over-replacement mirror hyperthyroidism: palpitations, tremor, heat intolerance, anxiety, weight loss and insomnia. A patient reporting these after a dose increase is describing an over-replacement problem, not a new diagnosis.
Pregnancy raises levothyroxine requirements
Thyroid hormone requirements increase during pregnancy, often early in the first trimester, and adequate maternal thyroid hormone is important for fetal neurodevelopment. A woman established on levothyroxine who becomes pregnant typically needs a dose increase and earlier, more frequent monitoring rather than continuing unchanged until her next routine review.
This is a high-yield stem shape because the instinct in pregnancy is often to reduce or stop medicines. Levothyroxine runs the other way: under-treatment is the risk to be avoided, and it is continued and usually increased.
Carbimazole and the agranulocytosis warning
Carbimazole is the usual first-line antithyroid drug in Australian practice. Its defining safety issue is a rare but serious risk of agranulocytosis, and the counselling that follows from it is specific: a patient developing a sore throat, mouth ulcers, fever or other signs of infection should seek medical attention promptly and have a full blood count taken.
This is not generic "report any side effects" advice — the point is that an ordinary-sounding sore throat in a patient on carbimazole is an urgent presentation rather than a minor complaint, and pharmacists are often the first point of contact for exactly that symptom.
In pregnancy the choice between the two antithyroid drugs reverses partway through. Propylthiouracil is generally preferred in the first trimester, because carbimazole carries a risk of specific congenital abnormalities. Beyond the first trimester, carbimazole is generally preferred again, because propylthiouracil carries a greater risk of serious hepatotoxicity — so neither drug is simply "the safer one", and which is appropriate depends on the stage of pregnancy.
Managing hyperthyroid symptoms while waiting for antithyroid therapy
Antithyroid drugs do not work immediately — it takes weeks for thyroid hormone levels, and therefore symptoms, to settle. In the meantime the patient is left with the adrenergic symptoms of thyrotoxicosis: tremor, palpitations, anxiety, heat intolerance, sweating and weight loss despite a normal or increased appetite.
A beta blocker, commonly propranolol, is used for symptomatic relief during that interval. The distinction OPRA tests is that the beta blocker does nothing to the underlying thyroid overactivity — it blunts the symptoms while the antithyroid drug does the actual work. A stem in which a patient is given a beta blocker alone and told their hyperthyroidism is being treated describes an incomplete regimen.
